- I confirm that the information provided is true and correct and I am aware that incorrect information can have a significant impact on the public health situation and the health of people in the workplace, their relatives and personal surroundings. I am aware that despite extensive protective measures and a hygiene concept, there is still a residual risk of getting infected with the SARS-CoV-2 while working. I take that risk

- If there are any changes to my state of health, I will inform my employer immediately
We process this data only for the purpose of minimizing the risk of infection with the corona virus by excluding potentially infected employees from work until a negative corona test is available. It is possible that a test will still be negative even though the person is already infected. If you suspect that you may have been infected, please call the doctor treating you or call 116 117. There it will be decided whether you need to be tested again.

To keep the risk of infection low, please continue to pay more attention to good hand hygiene.

Avoid unnecessary contact with other people and stay at home if possible. When you leave the house, keep a sufficient distance (at least 1.5 meters) from other people, wear a face mask where necessary and observe the general rules of conduct for protection against infection.
